What is Transition Management?

Transition management is a structured approach to help guide organizations in the planning and management of complex and critical organizational transitions associated with new construction, expansion, acquisitions, or other business structure changes. It combines project management and change management disciplines to address both the technical and people sides of transition.


Our team will work in partnership with your organization’s sponsors, key stakeholders, and frontline leaders to develop and deliver a customized transition strategy that addresses both building and people readiness.


Meet the RSA Transition Leaders

Karen Durano, MS, RN

Karen is a transition management consultant at Ruck-Shockey Associates, Inc. With over 20 of experience in healthcare operations, adult learning, and change management, she has managed and led multiple organizational transitions throughout her career.


Before joining Ruck-Shockey Associates, Karen developed and led transition strategies to support an academic medical center campus expansion that included the construction of new inpatient hospitals and outpatient treatment centers, and the implementation of a new Enterprise Resource Planning (ERP) software. Her focus on 'people readiness' assures that leaders and employees are engaged throughout the transition process and are prepared with the knowledge and skills needed to use new technology and adapt to changes in job roles, processes, and workflows. She understands the challenges and emotions associated with change and creates strategies to minimize and overcome resistance.


Karen holds a masters' degree in nursing from The Ohio State University and is Prosci Certified.

Robert Hofacre, BSN, MCRP, RN

Rob is a transition management consultant at Ruck-Shockey Associates, Inc. With over 20 years of experience in both inpatient and outpatient healthcare operations (Facilities, Environmental Services, and Equipment and Materials Management) and Nursing Services (Clinical, Management, and Administration).


Before joining Ruck-Shockey Associates, Rob developed and led transition strategies to support an academic medical center campus expansion that included the construction of new inpatient hospitals and outpatient treatment centers. his focus on 'building readiness' ensured that plan development, transition planning, equipment assessment/procurement, building fit-out, building/program activation, moves management, and post-occupancy reviews were comprehensive and completed timely within budget. He led multiple organizational transitions throughout his career. Rob also conducted Federal, State, County, and Military correctional facilities accreditation audits, Federal Court monitoring visits, and education compliance training, developed the Association's Accreditations Healthcare Standards, established monitoring criteria, and created outcome measures.


Rob holds a bachelor's degree in nursing from Michigan State University and a master's degree in City and Regional Planning from The Ohio State University.

During implementation, there is an active partnership with sponsors, key stakeholders, and frontline managers. The focus is on managing building and people readiness, including:

  • Building fit-out and activation

  • Go-live and post-occupancy support

Evaluation

The evaluation stage involves creating a summary report that includes:

  • Evaluation of transition goals and activities
  • Measurement of objective success parameters
